The clinical picture corresponds to seborrheic dermatitis in humans [1].

Several factors raise the risk of high Malassezia numbers. In a study of 98 dogs with various dermatoses, increased Malassezia counts were associated with seborrheic dermatitis, recent antibiotic treatment, and breed [7]. That means a dog recently treated with antibiotics for a skin infection can develop a yeast flare that looks like worsening seborrhea.

There is a histological clue as well. Subepidermal linear alignment of mast cells was found in 47.7% of dogs with Malassezia dermatitis in an earlier study, and a follow-up review of 419 biopsies found the pattern in only 0.7% of other inflammatory skin diseases [8]. This is a biopsy-level finding, not something an owner can see, but it supports the idea that Malassezia dermatitis is a distinct inflammatory pattern.

### Ectoparasites and Other Infections

Leishmaniasis is not a typical cause of scurf in most of the United States, but the case illustrates a general principle. Unusual infections can present as seborrhea, and a dog that does not respond to standard care needs a broader diagnostic workup.

### Primary and Hereditary Seborrhea

### Skin Scrapings and Cytology

Skin scrapings look for mites such as Demodex and Sarcoptes. Cytology, often done with tape strips or impression smears, quantifies bacteria and yeast. Trichography is microscopic examination of plucked hairs. It helps assess hair cycle abnormalities, follicle damage, and some infectious agents. It is a low-cost test that adds information when the coat is dull, thinning, or failing to regrow.

### Thyroid and Endocrine Testing

Thyroid testing is indicated when the history and physical exam suggest hypothyroidism, for example unexplained weight gain, lethargy, cold intolerance, or a nonpruritic symmetric pattern. ### When Biopsy Is Useful

### Antifungal and Antibacterial Shampoos

When Malassezia or bacteria are involved, the shampoo needs an antimicrobial ingredient. A randomized controlled trial compared two shampoos in 67 dogs with pedal or generalized dermatitis associated with Malassezia overgrowth. Dogs were treated with either a 3% chlorhexidine shampoo or a 2% miconazole plus 2% chlorhexidine shampoo. Among 54 dogs with good compliance, yeast counts dropped by at least 88% in 21 of 22 dogs on the chlorhexidine shampoo and 30 of 32 dogs on the combination shampoo. There was no significant difference between products for yeast reduction, time to cytological recovery, or lesion score improvement. Four dogs on the chlorhexidine shampoo had minor adverse effects [2].

The practical takeaway is that chlorhexidine-based bathing is effective for Malassezia overgrowth, and adding miconazole does not appear to improve outcomes in that trial. Your veterinarian will choose based on your dog's specific infection profile.

### Why Biofilms Matter for Stubborn Cases

Some cases resist antifungal treatment, and biofilm formation is one reason. A study of Malassezia pachydermatis and Candida parapsilosis from dog skin found that all single and dual cultures produced biofilms within 24 hours, with the greatest amount at 72 hours. Mixed cultures produced more biofilm than single strains. All sessile (biofilm) forms were resistant to itraconazole and ketoconazole, while planktonic forms were susceptible [14].

This is a laboratory finding, but it explains a clinical frustration. A yeast infection that looks like it should respond to an antifungal may persist because the organisms are organized in a biofilm. It also supports the value of mechanical disruption through bathing, since shampooing physically removes organisms and biofilm from the surface.

### Essential Fatty Acids and Skin Support

Essential fatty acid supplementation is a standard supportive measure for scaling disorders. It supports the skin barrier and can improve coat quality over time. It is not a substitute for treating an infection or an endocrine disease, but it is a reasonable adjunct in a comprehensive plan.

### Treating the Underlying Cause

This is where the real resolution happens.

- If allergy is the driver, the allergy must be managed. That may involve allergen avoidance, medication, or immunotherapy, and the plan is individualized.
- If hypothyroidism is the driver, thyroid supplementation can normalize the skin and skin flora, as shown in the early endocrine study [5].
- If hyperadrenocorticism is the driver, treating the endocrine disease is the priority.
- If Malassezia is the driver, antifungal therapy plus correcting the predisposing factor is needed.
- If parasites are the driver, targeted antiparasitic treatment is needed.

### What Does Not Work Well

Isotretinoin was studied for idiopathic seborrhea in dogs and was judged effective in only 1 of 8 dogs, with side effects including mild conjunctivitis, transient erythematous rash, and increased serum cholesterol and triglycerides [15]. That is a reminder that retinoid therapy is not a reliable first-line answer for most seborrheic dogs.

A case report described a spayed cocker spaniel with idiopathic chronic generalized seborrhea that responded poorly to medicated baths and intermittent antibiotics and steroids over 5.5 years, became refractory to steroid injections, did not benefit from oral vitamin A, and then recovered within a month on oral 13-cis-retinoic acid [16]. That is a single case, and it does not establish a general treatment recommendation. It does illustrate that truly idiopathic seborrhea can be difficult and may require referral-level management.

## Breed Predispositions to Know

Two breeds come up repeatedly in the seborrhea literature.

**Cocker spaniels** are the classic breed for primary idiopathic seborrhea. The keratinization studies were conducted in cocker spaniels [11][10], and the case report of refractory idiopathic seborrhea was in a cocker spaniel [16].

## Practical Care at Home

Owners can do a great deal to support a seborrheic dog between veterinary visits.

**Bathe on schedule, not occasionally.** Medicated bathing works only if it is done at the frequency your veterinarian recommends. Skipping baths allows scale, oil, and organisms to rebuild.

**Use the shampoo correctly.** Most medicated shampoos need contact time on the skin before rinsing. Lather, wait as directed, then rinse thoroughly. Residual shampoo can irritate skin.

**Do not substitute human dandruff products.** Human seborrheic dermatitis shampoos are formulated for human skin pH and human scalp biology. They are not designed for canine skin, and some ingredients are inappropriate or irritating for dogs. Use veterinary-recommended products.

**Keep ears and folds clean.** Greasy seborrhea concentrates in the ears, lip folds, facial folds, and tail folds. These areas need separate attention because shampoo alone may not reach them.

## What Is Still Uncertain

Primary seborrhea remains poorly understood at the molecular level. The keratinization studies show that epidermal renewal is altered in seborrheic cocker spaniels [10], but the underlying genetic and biochemical defect is not fully mapped.

The role of biofilms in clinical treatment failure is plausible and supported by laboratory data [14], but translating that into specific clinical protocols is still an active area.

The relationship between canine seborrheic dermatitis and human seborrheic dermatitis is conceptually useful [1], but the two conditions are not identical, and treatments do not transfer directly between species.

## Limitations and When to Contact a Veterinarian
