What Amoxicillin Is and What It Treats
Amoxicillin is a broad-spectrum aminopenicillin. It binds to proteins in the bacterial cell wall and prevents the wall from forming correctly. Bacteria that cannot maintain their cell wall either die or stop dividing. This is why amoxicillin works only against actively growing bacteria and why it does nothing for viral, fungal, or non-infectious causes of urinary signs.
In companion animal practice, amoxicillin and amoxicillin/clavulanic acid are used for a range of bacterial infections, including skin, soft tissue, dental, respiratory, and urinary infections. In the urinary tract specifically, the drug must reach the bladder and kidney in high enough concentrations to suppress the bacteria living there. Amoxicillin does concentrate in urine, which is one reason it has historically been used for bladder infections, but concentration alone does not overcome resistance.
A key point for owners is that a UTI in a dog or cat is not automatically a bacterial infection. In cats, bacterial UTI is one of the less common causes of lower urinary tract signs. Why Resistance Matters More Than Ever
Resistance is the central issue in deciding whether amoxicillin will work for a pet's UTI. The data from the past decade are consistent and sobering.
In a study of dogs and cats in Santiago, Chile, 52.4 percent of E. coli isolates were resistant to amoxicillin and 44.3 percent were resistant to amoxicillin/clavulanic acid [5]. In Norway, only 58 percent of feline urinary isolates were susceptible to amoxicillin, compared with 72 percent for amoxicillin/clavulanic acid [8]. When more than half of the most common uropathogen is resistant to a drug, giving that drug blindly is a coin flip at best. A urine culture tells the veterinarian which organism is present and which antibiotics will actually kill it. Susceptibility testing turns a guess into a decision.
Resistance also has a public health dimension. Extended-spectrum beta-lactamase (ESBL) producing E. coli can spread between pets in the same household and between pets and people. A case report documented the same CTX-M-14 ESBL-producing E. coli strain causing a urinary infection in a cat and, seven months later, a liver and bile duct infection in a dog living in the same home [9]. Using antibiotics only when they are needed and only when they are likely to work slows this kind of spread.
Amoxicillin vs Amoxicillin/Clavulanic Acid
Clavulanic acid is a beta-lactamase inhibitor. Many bacteria defend themselves against penicillins by producing enzymes called beta-lactamases that destroy the drug before it can act. Clavulanic acid blocks those enzymes, which restores amoxicillin's ability to kill bacteria that would otherwise resist it.
This combination is why amoxicillin/clavulanic acid is often chosen over plain amoxicillin for UTIs.
